Contrast-enhanced CT is the imaging modality of choice, allowing accurate grading per AAST guidelines and reducing the need for surgery. Conservative management—intravenous fluids, close monitoring, watchful waiting—is increasingly preferred, except in cases of extensive urinary extravasation, devitalized renal parenchyma, or associated abdominal injuries. A retrospective study indicated that primary conservative treatment leads to less surgery, less open surgery, and less blood and renal parenchyma loss compared to initial surgery. For athletes with a single kidney, the American Academy of Pediatrics now recommends a 'qualified yes' for contact/collision sports, though some physicians remain reluctant. Children's kidneys are more prone to injury due to size and location, but a study of 45,000 children found no kidney loss in contact sports, while sledding, skiing, and rollerblading did result in such loss.
