Blunt trauma, or blunt force trauma, happens when a strong impact hits the body without breaking the skin. This sets it apart from penetrating trauma, where an object pierces the skin and creates an open wound. Common causes include car crashes, assaults, sports injuries, and falls in older adults. The injuries that result can range from bruises, scrapes, and cuts to concussions, broken bones, and internal or external bleeding. How severe these injuries are depends on the force of the blow, which part of the body is hit, and any pre-existing health conditions the person has. In serious cases, blunt trauma can be life-threatening and needs urgent medical care. Death from blunt force trauma most often comes from head injuries or major blood loss.
Blunt abdominal trauma makes up about 75% of all blunt trauma cases and is the most common type. Most of these abdominal injuries—also about 75%—happen in motor vehicle crashes. In these crashes, rapid deceleration can throw a driver into the steering wheel, dashboard, or seatbelt. This can cause bruises in milder cases or, with more force, rupture internal organs due to a sudden spike in pressure inside them. At first, there may be few signs that serious internal damage has occurred, making diagnosis tricky and requiring a high level of suspicion. Two main physical forces are at work: compression and deceleration. Compression comes from a direct blow, like a punch, or from being pressed against a hard object like a seatbelt or steering column. This force can squash a hollow organ, raising its internal pressure and possibly causing it to burst. Deceleration, on the other hand, stretches and tears tissues where mobile organs in the abdomen, such as the bowel, are anchored. This can rip the mesentery—the tissue that holds the bowel in place—and damage the blood vessels inside it. Classic examples include a tear in the liver along the ligamentum teres and injuries to the renal arteries. When blunt abdominal trauma leads to internal injury, the liver and spleen are most often affected, followed by the small intestine.
